How to Make Referral

ANYONE & EVERYONE can submit a referral!

Step 1: Go to SHOP55.ORG and click the link for the online referral form.

Step 2: Fill in required info

             - Fill in as much as you can

             - The more info, the better we can understand the situation.

Step 3: Click 'SUBMIT' AND you're done!

*Reminder: After you click 'Submit' you have options to edit or submit another referral.

1-2 Engagement sessions & conduct RAAPS survey with Shop 55 screening team
SELF Referral
Connect to appropriate provider on campus or off campus
Use COST team meetings to coordinate support services for student(s)
Connect student to service(s) & status update at COST

What is COST?

The Coordination of Services Team (COST) is the learning support umbrella structure that brings together all support service providers at a school site. The COST develops and monitors programs and resources that enhance prevention and intervention efforts contributing to student success.

COST meets every Thursday morning in the Wellness Center.
